Arterial Pulsatility and Aortic Valve Function under Continuous Flow Left Ventricular Assist
نویسنده
چکیده
Continuous Flow Left Ventricular Assist Devices (CF-LVADs) generally operate at a constant speed, which causes a decrease in pulse pressure and pulsatility in the arteries and allegedly may lead to late complications such as aortic insufficiency and gastrointestinal bleeding. The purpose of this study is to increase the arterial pulse pressure and pulsatility while obtaining more physiological hemodynamic signals, by controlling the CF-LVAD flow rate. A lumped parameter model was used to simulate the cardiovascular system including the heart chambers, heart valves, systemic and pulmonary arteries and veins. A baroreflex model was used to regulate the heart rate and a model of the Micromed DeBakey CF-LVAD to simulate the pump dynamics at different operating speeds. A model simulating the flow rate through the aortic valve served as reference model. CF-LVAD operating speed was regulated by applying proportional-integral (PI) control to the pump flow rate. For comparison, the CF-LVAD was also operated at a constant speed, equaling the mean CF-LVAD speed as applied in pulsatile mode. In different operating modes, at the same mean operating speeds, mean pump output, mean arterial pressure, end-systolic and end-diastolic volume and heart rate were the same over the cardiac cycle. However, the arterial pulse pressure

Comparison of continuous-flow and pulsatile-flow left ventricular assist devices: is there an advantage to pulsatility?
BACKGROUND Continuous-flow left ventricular assist devices (CFVAD) are currently the most widely used type of mechanical circulatory support as bridge-to-transplant and destination therapy for end-stage congestive heart failure (HF). Compared to the first generation pulsatile-flow left ventricular assist devices (PFVADs), CFVADs have demonstrated improved reliability and durability. However, CF...

Continuous-flow left ventricular assist devices (LVAD) have become standard therapy option for patients with advanced heart failure. They offer several advantages over previously used pulsatile-flow LVADs, including improved durability, less surgical trauma, higher energy efficiency, and lower thrombogenicity. These benefits translate into better survival, lower frequency of adverse events, imp...

BACKGROUND A continuous flow (CF) left ventricular assist device (LVAD) has various advantages over a pulsatile flow (PF) LVAD, but the extent of preventing aortic insufficiency (AI) by each type of LVAD remains controversial. METHODSANDRESULTS Of 86 patients with non-ischemic cardiomyopathy who underwent LVAD implantation between 2006 and 2015, 20 propensity score-matched patients with PF LV...

BACKGROUND Bleeding is an important cause of morbidity and mortality in patients with continuous-flow left ventricular assist devices (LVADs). Reduced pulsatility has been implicated as a contributing cause. The aim of this study was to assess the effects of different degrees of pulsatility on the incidence of nonsurgical bleeding. METHODS AND RESULTS The Utah Transplantation Affiliated Hospi...
